Boston Scientific called 2027 the trough and guided Q4 sales lower, but at 14× earnings with the CEO buying, the stock is priced for most of that bad news

Q3 2026 sales rose only 1.5% to $5.14B, roughly in line with consensus, after the 25 August cyberattack cost ~300 bps of growth and ~$0.07 of EPS. Adjusted EPS was $0.73, below the company's own $0.80–0.82 guide. Q4 sales are now guided to fall 1–3%, and management expects 2027 to be "the trough", with electrophysiology and WATCHMAN still losing ground. The stock, already down 53% this year, traded ~8% lower at ~$41.64. Over the next 10–30 days the bad news is largely out; the question is whether a 14× multiple and insider buying can hold a floor before there is any growth to point to.

ProfitabilityAdjusted EPS $0.73 against $0.75 a year ago; vendor consensus $0.72, other polls $0.75, company guide $0.80–0.82. Adjusted gross margin 68.0% (−300 bps), adjusted operating margin 26.5% (−150 bps). GAAP operating margin 16.1% against 20.7%, after $81M of litigation and $63M of restructuring charges.Mixed
Valuation vs peers14.1× TTM adjusted EPS ($3.19) and 14.2× the $3.17 2026 guide midpoint. GAAP P/E 19.0× against a 29.8× peer median; P/S 3.09× against 4.23×; EV/EBITDA ~13×. Consensus target $57.82 (30 analysts, pre-print), 28% above the close.Bullish
Platform KPIsFARAPULSE is losing PFA share, and the next-generation FARAWAVE Ultra has slipped to early 2028. US standalone WATCHMAN procedures fell in the high teens, while concomitant procedures grew ~35%; a new WATCHMAN competitor is coming. Neuromodulation +13% and LACA +10.5% operational were the bright spots.Bearish
Balance sheetDebt $12.6B against cash of $0.5–0.6B (Jun–Sep); gross leverage 1.9×. Q3 FCF $867M. Share count down to ~1.45B from 1.49B, with buybacks a stated priority. The Penumbra acquisition now closes in early 2027, and its dilution will be larger than the original $0.06–0.08.Mixed
Regime stateQ3 Quiet Range: Persistency −0.130 (50.5th percentile; negative throughout the window), Volatility −0.424 (7.6th). Held 25 sessions since 23 Sep. BSX has alternated only between Quiet Range and Volatile Chop for 14 months, both mean-reverting. Not borderline. As of 27 Oct 2026 (1 trading day lag).Neutral
Driver exposure90.1% idiosyncratic. Sector Driver 1 −0.314 (R² 9.9%); Market Driver 1 ~0. One-year correlation with the S&P 500 is just 0.05 (beta 0.16); with Medtronic 0.47 and Stryker 0.42. Company news has decided every move this year. As of 27 Oct 2026.Bullish
Key risk (next 10–30 days)Post-print estimate cuts. Analysts must lower Q4 and 2027 numbers, and recent targets cluster at $48–56. After three of the last five prints the stock fell a further 5–12% over the next 21 sessions. Peer and conference data (Medtronic ~18 Nov, AHA mid-November) may show further share loss in PFA and left atrial appendage closure.Bearish
Catalysts in windowJust before: FOMC (28 Oct) and the 10-Q (early Nov, expected). Inside: AHA Scientific Sessions (mid-Nov, expected), Medtronic Q2 FY27 results (~18 Nov, expected) and options expiry (20 Nov). The next BSX report (~early Feb 2027) and the Penumbra close (early 2027) are outside.Mixed
Overall view (10–30 days)Mixed. The fundamental picture got worse: Q4 sales will fall, 2027 will be flat, and the two growth engines are losing share. But the stock has already lost more than half its value, trades at ~14× earnings against peers near 30×, and the CEO bought $9.0M of stock at $48 in August. With a mean-reverting regime, a favourable market map and almost no index linkage, a range near the lows is more likely than either a recovery or a collapse. Composite 6.4 / 10. Mixed

Regime and driver readings as of 27 Oct 2026, 1 trading day behind the report date; market regimes as of 26 Oct 2026, 2 trading days behind. The release came before the open on 28 Oct; the session was still trading when this report was written (~$41.64 at 10:10 ET). Revenue consensus: StockStory ($5.07B) and Investing.com (~$5.12B). Cyber impact, EP, WATCHMAN and margin detail from the Investing.com call transcript.

Boston Scientific has gone from a growth leader to a turnaround in nine months. Management has now said 2027 is the trough, which caps how bad the guidance can get from here but gives the stock nothing to rally on. For the next month the multiple and insider buying should support the price, while estimate cuts and peer data keep a lid on it.

01

Investment Thesis

Each case states what would have to happen inside the next 10–30 days for it to play out. This section argues; the verdict is in section 02.

Bull case
Kitchen-sink guide. Management called 2027 the trough and guided Q4 below consensus by ~4.5%; the reset is out in one go.
Cheap. ~14× adjusted EPS against peers near 30× GAAP, and 3.1× sales against 4.2×, for a business with a 68% gross margin and 26.5% adjusted operating margin.
Insiders are buying. The CEO bought 186,240 shares at $48.33 ($9.0M) on 3 Aug; six directors bought between May and August at $45–57.
The cyber hit is temporary. Operations were fully restored by 8 Sep; ~$150M of Q3 sales and $0.07 of EPS were lost, with a smaller 200–300 bps drag in Q4 as customer stock unwinds.
Favourable market map. US market, SP500 and Global market are all in Quiet Drift, BSX's best regime for each (Sharpe 1.21, 0.81, 1.92).
What must happen in the window: estimate cuts are absorbed quickly, peer data show stable markets, and BSX closes back above $48.33, the CEO's purchase price.
Neutral case
A floor without a catalyst. Valuation and insider buying limit the downside, but nothing inside the window can show growth returning.
Mean-reverting behaviour. Persistency has been negative for the whole 14-month window; moves have tended to reverse rather than extend.
A modest band. At 24.5% annualised volatility, a ±1 standard-deviation 21-session move from $41.64 is about ±$2.9, which spans roughly $38.70–44.60.
Targets near the price. Recent targets from Barclays, Citi and Wells Fargo ($48) and Truist ($51) sit 15–22% above today's price; cuts will narrow the gap.
What must happen in the window: no new negative data, and BSX holds between ~$39.50 and ~$45.
Bear case
Growth has stopped. +1.5% in Q3, −1% to −3% guided for Q4, flat 2027 excluding Penumbra, down from +20% a year ago.
Both franchises are losing share. EP −5% (US −8%) as rival PFA systems gain; WATCHMAN −6% with standalone US procedures down in the high teens and a new competitor coming.
Pipeline slippage. FARAWAVE Ultra moved to early 2028; analysts on the call focused on further pipeline delays.
Penumbra dilution. Closing slipped to early 2027, and higher rates will make it more dilutive than the original $0.06–0.08.
Post-print drift. After the Q4 2025 and Q1 2026 prints the stock fell another 5.5% and 11.9% over the next 21 sessions.
What must happen in the window: analysts cut 2027 EPS sharply, Medtronic or AHA data show more share loss, and BSX breaks today's $39.57 low.
Boston Scientific has told investors the worst year is 2027. The stock already trades as if it believes that; what it cannot yet show is the recovery after it.
02

Composite Assessment

The finding: over the next 10–30 days, the cut to a flat 2027 is offset by a 14× multiple and heavy insider buying, so a range near the lows is the most likely outcome. Only a close above $48.33 would favour the bull case; a break below $39.57 would mean estimate cuts have overwhelmed the valuation support.

2.2 — Where it wins and where it loses

Wins
Neuromodulation. +13.0% organic to $331M.
International. EMEA +6.1% and LACA +10.5% operational; international EP +2%, WATCHMAN +4%.
Concomitant WATCHMAN. Procedures done alongside ablation grew ~35%.
Cash. Q3 free cash flow $867M despite the outage.
Pipeline and reimbursement. Elite AF trial enrolling; new CPT codes for renal denervation and tibial nerve stimulation from 2028.
Loses
US electrophysiology. −8%; about half the miss was the cyberattack, the rest share loss.
US WATCHMAN. −7%; standalone procedures down in the high teens.
Gross margin. Adjusted 68.0%, −300 bps; GAAP 66.7% against 69.9%.
Urology. −3.1% organic; Endoscopy −0.6%.

03

Risk Factors

Ordered by what can bite inside the next 10–30 days, not by ultimate severity. Structural risks that cannot resolve inside the window are marked as context.

Live in the window
Estimate and target cuts. Q4 sales guidance sits ~4.5% below consensus, and management expects flat 2027 EPS excluding Penumbra. Targets of $48–56 from Barclays, Citi, Wells Fargo, Truist and Stifel will move lower.
Competitive data at AHA (mid-November). Rival PFA and left atrial appendage closure data could add to the share-loss narrative; Bernstein already cited Abbott's Amulet 360 as a threat.
Medtronic's results (~18 Nov). Medtronic is the most correlated peer (0.47) and a direct PFA competitor; strong cardiac ablation growth there would confirm BSX's share loss.
Lingering cyber effects. Q4 still carries a 200–300 bps drag as customers work down stock built after the outage; investigation findings or litigation could follow.
Structural context
PFA market maturity. PFA is ~80% of US AFib ablation revenue, so new users no longer offset rivals' share gains; FARAWAVE Ultra is not due until early 2028.
WATCHMAN demand. New stroke-risk evidence changed referral patterns, shifting procedures to concomitant cases and reducing standalone implants.
Penumbra integration. A large acquisition closing in early 2027 with higher financing costs than planned.
China pricing. Volume-based procurement pressure cited for 2027.

Peer TTM EPS is TTM net income over vendor shares (derived). PEG uses latest-quarter net income growth; n/m where growth is negative.

Boston Scientific has gone from the most expensive name in large-cap medtech to the cheapest on sales and among the cheapest on earnings. The discount is earned by a two-year growth gap, not by weak margins: its gross and operating margins are still at or above the peer median.

Over the window, 21-day forward returns from Quiet Range had a median of −5.3% (28% positive, 124 observations) and from Volatile Chop −7.6% (22%, 144). Unconditionally the median was −6.7% (25% positive), in a stock that fell ~58% over the window, so neither state has favoured holding. As of 27 Oct 2026.

Persistency moves slowly.

The series moves in small daily steps and can hold near one value for weeks, producing near-vertical runs on the trace. That is the data behaving normally. Boston Scientific has stayed on the mean-reverting side all window, alternating between quiet ranges and volatile chop as each piece of bad news arrived. These statistics describe 4 Sep 2025 – 27 Oct 2026, a window dominated by a 58% decline, and are not predictions.

BSX daily returns bucketed by each market group's regime quadrant. Window 8 Feb 2022 – 26 Oct 2026, 1,183 overlapping days. No bucket falls below 30 days (the smallest is 70), so none is marked thin. All 14 mapped groups. Groups in bold have Sharpe spreads above 1.5, meaning BSX is materially sensitive to that group's regime. Sharpe spread is best-regime Sharpe minus worst-regime Sharpe. Market regimes as of 26 Oct 2026, 2 trading days behind.

12.3 — Sensitivity callouts

Most sensitive to the Gold regime (Sharpe spread 5.90). Gold has been in Volatile Chop for 58 days, BSX's worst Gold state (Sharpe −1.03). Bonds long (4.85) and VIX Near (4.44) are next; both sit in regimes with a positive BSX Sharpe (0.65 and 2.04).
What has historically followed the current US market regime over 10–30 days. In the 288 days the US market was in Quiet Drift with a full 21-day window, BSX's 21-day forward return had a median of +1.7% and was positive 64.2% of the time. Unconditionally the median was +1.3% and positive 56.8% of the time. For SP500 Quiet Drift the figures were +2.9% and 71.0%. This is history, not a forecast.
A favourable configuration. 9 of 14 groups sit in a regime with a positive BSX Sharpe, and US market (1.21), SP500 (0.81) and Global market (1.92) are all in Quiet Drift, BSX's best state for each. Four sit in its worst: Technology Quiet Range (−0.42), Gold (−1.03), Bonds near (−1.10) and Bonds mid (−0.63).
The map has not helped in 2026. The favourable history comes mostly from BSX's 2022–25 growth era; this year company news (correlation with the S&P 500 of 0.05) has overwhelmed the market backdrop. The favourable map is a modest tailwind, not a reason to expect a rebound.
Do not trade this table.

Regime-conditional history describes 8 Feb 2022 – 26 Oct 2026, not the future. No regime bucket here holds fewer than 30 days; where one does, annualised figures should not be relied on. Overlapping forward windows overstate the independence of observations, and most of this history comes from BSX's growth era, which has ended. Market regime series as of 26 Oct 2026, 2 trading days behind the report date.

Overall view · next 10–30 days
Mixed · cheap at the trough, no catalyst yet

Boston Scientific's third quarter showed a business that has stopped growing: sales +1.5%, with electrophysiology −5% and WATCHMAN −6%. Q4 sales are guided to fall, and management now calls 2027 the trough, with growth not returning until 2028. The cyberattack explains part of 2026 but not the share loss. The stock already reflects much of this: down 53% this year, ~14× adjusted earnings against peers near 30×, and the CEO bought $9.0M of stock at $48 in August. It is a company-specific name in a mean-reverting state, with no scheduled catalyst of its own before February. The view turns bullish if estimate cuts are absorbed and BSX closes above $48.33, the CEO's purchase price. It turns bearish if competitor data at AHA or from Medtronic deepen the share-loss story and the stock breaks today's $39.57 low.
